Problem to create new user account in admin panel

After the auto update upgraded to 1.0.41, I am having problem to create new user account in admin panel.

I went to the Users listing page, click the button "Add New User", fill in all the details and choose the package, hit the button "Create Account". The page show success message.

But the User listing page didn't show the new user account that I just created.

I have tried logout, and different internet browser, still the same issue. I have tried to create the user account with different details, also same problem.

Conclusion, I can't create any new user account right now.

I am using Linux server.
